Case Management
The goal of case management is to give seniors and their families the help they need to remain in their homes and communities as they age. After a case manager completes an initial assessment,
they will see what services a senior and their family require. That may include transportation to get to stores and doctor's appointments, home-delivered meals, personal care services, help
with general and heavy household chores, and respite care services for overburdened caregivers.
Those services will then be provided directly by Bridgeway employees, or by carefully screened partner companies and organizations.
